El manejo del paciente pediátrico con falla respiratoria (FR) en las unidades de cuidados intensivos requiere de manejo especializado, y un soporte ventilatorio oportuno que permita el mejoramiento y la estabilización de la función pulmonar mediante la presión positiva generada por la ventilación mecánica invasiva. Los modos ventilatorios convencionales permiten el apoyo respiratorio mediante la aplicación de presiones elevadas en la vía aérea, lo cual permite la generación de volúmenes corrientes (Vt) logrando así un apropiado intercambio gaseoso. Es importante anotar que la ventilación convencional debido a las presiones generadas en la vía aérea lesionan el pulmón y pueden estar asociadas a Vt o presiones altas. En la actualidad el manejo de la ventilación mecánica está encaminada a disminuir la lesión pulmonar siendo la ventilación de alta frecuencia un modo no convencional que ayuda a disminuir la lesión pulmonar en los pacientes con FR.
